Association of Longer Leukocyte Telomere Length With Cardiac Size, Function, and Heart Failure

Is leukocyte telomere length (LTL) associated with alterations in cardiovascular structure and function? In this cross-sectional study including 40 459 UK Biobank participants, longer LTL was associated with higher left ventricular mass, larger ventricular and atrial sizes, and higher stroke volumes. Mendelian randomization analysis demonstrated a potential causal genetic association between LTL and left ventricular mass, ventricular size, and left ventricular stroke volume, and longer LTL was associated with a lower risk of incident heart failure after accounting for potential confounders.

Short telomere syndromes are the most prevalent premature aging disorders, with prominent phenotypes affecting the lung and hematopoietic system. Less understood are a newly recognized group of cancer-prone syndromes that are associated with mutations that lengthen telomeres. A large body of new data from Mendelian genetics and epidemiology now provides an opportunity to reconsider paradigms related to the role of telomeres in human aging and cancer.


Telomere Length and Clonal Hematopoiesis.

A recent study proposes a key role for telomere maintenance in the development of clonal hematopoiesis. Some persons with clonal hematopoiesis are at increased risk for the development of myeloid cancers such as acute myeloid leukemia or myelodysplastic syndromes, a risk that increases as the hematopoietic clone expands in size.16 Stopping this expansion may delay or avert leukemic progression, and therapeutic approaches to this end are being developed and tested.


Familial Clonal Hematopoiesis in a Long Telomere Syndrome.

A total of 17 POT1 mutation carriers and 21 noncarrier relatives were initially included in the study, and a validation cohort of 6 additional mutation carriers was subsequently recruited. A majority of the POT1 mutation carriers with telomere length evaluated (9 of 13) had long telomeres (>99th percentile). POT1 mutation carriers had a range of benign and malignant neoplasms involving epithelial, mesenchymal, and neuronal tissues in addition to B- and T-cell lymphoma and myeloid cancers.


Link Between Long Telomeres and Long Life Is a Tall Tale, Study Finds

The longer a person’s telomeres, researchers found, the greater the risk of cancer and other disorders, challenging a popular hypothesis about the chromosomal roots of vitality. While short telomeres do lead to health problems, long telomeres lead to health problems of their own. Far from extending life, long telomeres appear to cause cancer and a blood disorder known as CHIP, a condition that increases the risk of blood cancers and heart disease.


Genetic and Clinical Determinants of Telomere Length

Our GWAS confirmed 11 genetic loci previously associated with leucocyte telomere length (LTL) and two novel loci in SCNN1D and PITPNM1. PheWAS of LTL identified 67 distinct clinical phenotypes associated with both short and long LTL. We demonstrated that several diseases associated with LTL were related to one another but were largely independent from LTL genetics. Age of death was correlated with LTL independent of age.

Numerous factors localize at telomeres to regulate their length, structure and function, to avert replicative senescence or genome instability and cell death. In humans, Mendelian defects in several of these factors can result in abnormally short or dysfunctional telomeres, causing a group of rare heterogeneous premature-ageing diseases, termed telomeropathies, short-telomere syndromes or telomere biology disorders (TBDs). Here, we review the TBD-causing genes identified so far and describe their main functions associated with telomere biology.


Association of Newborn Telomere Length With Blood Pressure in Childhood.

In this birth cohort study including 485 newborns, participants with a longer birth TL had a significantly lower diastolic and mean arterial pressure at the age of 4 to 6 years. Furthermore, longer newborn TL was significantly associated with lower odds of having high childhood blood pressure. These results suggest that TL at birth associates with early life vascular health and that newborn TL may be a programmed molecular mediator containing genetic and prenatal environmental exposure information contributing to vascular health later in life.


Polygenic basis and biomedical consequences of telomere length variation

We characterize the genetic architecture of naturally occurring variation in leukocyte telomere length (LTL) and identify causal links between LTL and biomedical phenotypes in 472,174 well-characterized UK Biobank participants. We identified 197 independent sentinel variants associated with LTL at 138 genomic loci (108 new). Genetically determined differences in LTL were associated with multiple biological traits, ranging from height to bone marrow function, as well as several diseases spanning neoplastic, vascular and inflammatory pathologies. We estimated that, at the age of 40?years, people with an LTL >1?s.d. shorter than the population mean had a 2.5-year-lower life expectancy compared with the group with =1?s.d. longer LDL.


Genomic and evolutionary classification of lung cancer in never smokers

Whole-genome sequencing of 232 LCINS showed 3 subtypes defined by copy number aberrations. The dominant subtype (piano), which is rare in lung cancer in smokers, features somatic UBA1 mutations, germline AR variants and stem cell-like properties, including low mutational burden, high intratumor heterogeneity, long telomeres, frequent KRAS mutations and slow growth, as suggested by the occurrence of cancer drivers’ progenitor cells many years before tumor diagnosis. The other subtypes are characterized by specific amplifications and EGFR mutations (mezzo-forte) and whole-genome doubling (forte). No strong tobacco smoking signatures were detected, even in cases with exposure to secondhand tobacco smoke.

A constellation of bone marrow failure, pulmonary fibrosis, liver cirrhosis and premature greying is suggestive, however incomplete penetrance results in highly variable manifestations, with idiopathic pulmonary fibrosis as the most common presentation. Currently, the true extent of TBD burden is unknown as there is no established diagnostic criteria and the disorder often is unrecognized.

This cohort study included 3259 participants from 3 longitudinal studies, of whom 1525 died during the follow-up period. Leukocyte telomere length–associated mortality from noncancer causes increased as participants aged, approaching their age at death. These data suggest that leukocyte telomere length is associated with a life span limit among contemporary humans.

A population of cells in individuals with a history of smoking had mutational burdens that were equivalent to those expected for people who had never smoked: these cells had less damage from tobacco-specific mutational processes, were fourfold more frequent in ex-smokers than current smokers and had longer telomeres than their more-mutated counterparts.
