Understanding myalgic encephalomyelitis.

An important research challenge is the lack of a validated biomarker, laboratory-based test, and animal model, likely attributed to inconsistency in protocols, such as cell isolation, sample type, and technique. These challenges provide important lessons for research of Long Covid. By developing interdisciplinary and consistent research protocols, the pathomechanism of ME/CFS and Long Covid can be elucidated.


Genetic Risk Factors for ME/CFS Identified using Combinatorial Analysis

We applied both GWAS and the PrecisionLife combinatorial analytics platform to analyze ME/CFS cohorts from UK Biobank, including the Pain Questionnaire cohort, in a case-control design with 1,000 cycles of fully random permutation. The results from this study were supported by a series of replication and cohort comparison experiments, including use of a disjoint Verbal Interview cohort also derived from UK Biobank, and results compared for reproducibility. Results: Combinatorial analysis revealed 199 SNPs mapping to 14 genes, that were significantly associated with 91% of the cases in the ME/CFS population.


Redox imbalance links COVID-19 and myalgic encephalomyelitis/chronic fatigue syndrome

We summarize the evidence that people with acute COVID-19 and with ME/CFS have biological abnormalities including redox imbalance, systemic inflammation and neuroinflammation, an impaired ability to generate adenosine triphosphate, and a general hypometabolic state. We also examine the bidirectional relationship between redox imbalance, inflammation, energy metabolic deficits, and a hypometabolic state. We speculate as to what may be causing these abnormalities. Thus, understanding the molecular underpinnings of both PASC and ME/CFS may lead to the development of novel therapeutics.

In deciding how to act on long COVID, we must take heed of what happened in the case of myalgic encephalomyelitis, also called chronic fatigue syndrome (ME/CFS). The condition shares some of the symptoms of long COVID, and people with ME/CFS struggled for years to be recognized as having a serious medical condition that needed specialized treatment and research.
