Reducing dust and respirable crystalline silica near conveyors using a hybrid dust control system

Min Metall & Explor 2024 Occupational exposures to respirable dusts and respirable crystalline silica (RCS) is well established as a health hazard in many industries including mining, construction, and oil and gas extraction. The U.S. National Institute for Occupational Safety and Health (NIOSH) is researching methods of controlling fugitive dust emissions at outdoor mining operations. In this study, a prototype engineering control system to control fugitive dust emissions was developed combining passive subsystems for dust settling with active dust filtration and spray-surfactant dust suppression comprising a hybrid system. The hybrid system was installed at an aggregate production facility to evaluate the effectiveness of controlling fugitive dust emissions generated from two cone crushers and belt conveyors that transport crushed materials. To evaluate effectiveness of the system, area air measurements (n = 14 on each day for a total of 42 samples) for respirable dust were collected by NIOSH before, during, and after the installation of the dust control system in the immediate vicinity of the crushers and the nearby conveyor transfer point. Compared to pre-intervention samples, over short periods of time, geometric mean concentrations of airborne respirable dust were reduced by 37% using passive controls (p = 0.34) but significantly reduced by 93% (p < 0.0001) when the full hybrid system was installed. This proof-of-concept project demonstrated that the combined use of active and passive dust controls along with a spray surfactant can be highly effective in controlling fugitive dust emissions even with minimal use of water, which is desirable for many remote mining applications. © This is a U.S. Government work and not under copyright protection in the US; foreign copyright protection may apply 2024. |

Characterizing the etiology of recurrent tuberculosis using whole genome sequencing-Alaska, USA, 2008-2020

J Infect Dis 2024 BACKGROUND: Understanding the etiology of recurrent tuberculosis (rTB) is important for effective TB control. Prior to the advent of whole genome sequencing (WGS), attributing rTB to relapse or reinfection using genetic information was complicated by the limited resolution of conventional genotyping methods. METHODS: We applied a systematic method of evaluating whole genome single nucleotide polymorphism (wgSNP) distances and results of phylogenetic analyses to characterize the etiology of rTB in American Indian and Alaska Native (AIAN) persons in Alaska during 2008-2020. We contextualized our findings through descriptive analyses of surveillance data and results of a literature search for investigations that characterized rTB etiology using WGS. RESULTS: The percentage of TB cases in AIAN persons in Alaska classified as recurrent episodes (11.8%) was three times the national percentage (3.9%). Of 38 recurrent episodes included in genetic analyses, we attributed 25 (65.8%) to reinfection based on wgSNP distances and phylogenetic analyses; this proportion was the highest among 16 published point estimates identified through the literature search. By comparison, we attributed 11 of 38 (28.9%) and 6 of 38 (15.8%) recurrent episodes to reinfection based on wgSNP distances alone and on conventional genotyping methods, respectively. CONCLUSIONS: WGS and attribution criteria involving genetic distances and patterns of relatedness can provide an effective means of elucidating rTB etiology. Our findings indicate that rTB occurs at high proportions among AIAN persons in Alaska and is frequently attributable to reinfection, reinforcing the importance of active surveillance and control measures to limit the spread of TB disease in Alaskan AIAN communities. |
Racial and ethnic disaggregation of tuberculosis incidence and risk factors among American Indian and Alaska native persons-United States, 2001-2020

To examine impacts of racial and ethnic disaggregation on the characterization of tuberculosis (TB) epidemiology among American Indian and Alaska Native (AI/AN) persons in the United States. Methods. Using data reported to the National Tuberculosis Surveillance System during 2001 to 2020, we compared annual age-adjusted TB incidence and the frequency of TB risk factors among 3 AI/AN analytic groups: non-Hispanic AI/AN alone persons, multiracial/Hispanic AI/AN persons, and all AI/AN persons (aggregate of the first 2 groups). Results. During 2009 to 2020, annual TB incidence (cases per 100 000 persons) among non-Hispanic AI/AN alone persons (range = 3.87-8.56) was on average 1.9 times higher than among all AI/AN persons (range = 1.89-4.70). Compared with non-Hispanic AI/AN alone patients with TB, multiracial/Hispanic AI/AN patients were significantly more likely to be HIV positive (prevalence ratio [PR] = 2.05) and to have been diagnosed while a resident of a correctional facility (PR = 1.71), and significantly less likely to have experienced homelessness (PR = 0.53) or died during TB treatment (PR = 0.47). Conclusions. Racial and ethnic disaggregation revealed significant differences in TB epidemiology among AI/AN analytic groups. Exclusion of multiracial/Hispanic AI/AN persons from AI/AN analytic groups can substantively affect estimates of racial and ethnic health disparities. Using geographic disaggregation to compare tuberculosis epidemiology among American Indian and Alaska native persons-USA, 2010-2020

J Racial Ethn Health Disparities 2024 BACKGROUND: American Indian and Alaska Native (AIAN) populations are frequently associated with the highest rates of tuberculosis (TB) disease of any racial/ethnic group in the USA. We systematically investigated variation in patterns and potential drivers of TB epidemiology among geographically distinct AIAN subgroups. METHODS: Using data reported to the National Tuberculosis Surveillance System during 2010-2020, we applied a geographic method of data disaggregation to compare annual TB incidence and the frequency of TB patient characteristics among AIAN persons in Alaska with AIAN persons in other states. We used US Census data to compare the prevalence of substandard housing conditions in AIAN communities in these two geographic areas. RESULTS: The average annual age-adjusted TB incidence among AIAN persons in Alaska was 21 times higher than among AIAN persons in other states. Compared to AIAN TB patients in other states, AIAN TB patients in Alaska were associated with significantly higher frequencies of multiple epidemiologic TB risk factors (e.g., attribution of TB disease to recent transmission, previous diagnosis of TB disease) and significantly lower frequencies of multiple clinical risk factors for TB disease (e.g., diagnosis with diabetes mellitus, end-stage renal disease). Occupied housing units in AIAN communities in Alaska were associated with significantly higher frequencies of multiple measures of substandard housing conditions compared to AIAN communities in other states. CONCLUSIONS: Observed differences in patient characteristics and substandard housing conditions are consistent with contrasting syndromes of TB epidemiology in geographically distinct AIAN subgroups and suggest ways that associated public health interventions could be tailored to improve efficacy. |
Role of pre-farrow natural planned exposure of gilts in shaping the passive antibody response to rotavirus a in piglets

However, the efficacy of NPE protocols in generating lactogenic immunity has not been investigated before. A longitudinal study was conducted to investigate the dynamics of genotype-specific antibody responses to different doses (3, 2 and 1) of Rotavirus A (RVA) NPE (genotypes G4, G5, P[7] and P[23]) in gilts and the transfer of lactogenic immunity to their piglets. Group 1 gilts received three doses of NPE at 5, 4 and 3 weeks pre-farrow (WPF), group 2 received two doses at 5 and 3 WPF, group 3 received one dose at 5 WPF, and group 4 received no NPE (control group). VP7 (G4 and G5) and truncated VP4* (P[7] and P[23]) antigens of RVA were expressed in mammalian and bacterial expression systems, respectively, and used to optimize indirect ELISAs to determine antibody levels against RVA in gilts and piglets. In day-0 colostrum samples, group 1 had significantly higher IgG titers compared to the control group for all four antigens, and either significantly or numerically higher IgG titers than groups 2 and 3. Group 1 also had significantly higher colostrum IgA levels than the control group for all antigens (except G4), and either significantly or numerically higher IgA levels compared to groups 2 and 3. In piglet serum, group 1 piglets had higher IgG titers for all four antigens at day 0 than the other groups. Importantly, RVA NPE stimulated antibodies in all groups regardless of the treatment doses and prevented G4, G5, P[7] and P[23] RVA fecal shedding prior to weaning in piglets in the absence of viral challenge. The G11 and P[34] RVA genotypes detected from pre-weaning piglets differed at multiple amino acid positions with parent NPE strains. In conclusion, the results of this study suggest that the group 1 NPE regimen (three doses of NPE) resulted in the highest anti-RVA antibody (IgG and IgA) levels in the colostrum/milk, and the highest IgG levels in piglet serum. |

Lab on a chip for detecting Clara cell protein 16 (CC16) for potential screening of the workers exposed to respirable silica aerosol

Microfluid Nanofluidics 2023 27 (11) 1-10 Early detection of pulmonary responses to silica aerosol exposure, such as lung inflammation as well as early identification of silicosis initiation, is of great importance in disease prevention of workers. In this study, to early screen the health condition of the workers who are exposed to respirable silica dusts, an immunoassay lab on a chip (LOC) was designed, developed and fully characterized for analyzing Clara cell protein 16 (CC16) in serum which has been considered as one of the potential biomarkers of lung inflammation or lung damage due to the respirable silica dusts. Sandwich immunoassay of CC16 was performed on the LOC developed with a custom-designed portable analyzer using artificial serums spiked with CC16 protein first and then human serums obtained from the coal mine workers exposed to the respirable silica-containing dusts. The dynamic range of CC16 assay performed on the LOC was in a range of 0.625–20 ng/mL, and the achieved limit of detection (LOD) was around 0.35 ng/mL. The assay results of CC16 achieved from both the developed LOC and the conventional 96 well plate showed a reasonable corelation. The correlation between the conventional reader and the developed portable analyzer was found to be reasonable, resulting in R 2 ~ 0.93. COVID-19 pandemic impact on United States intimate partner violence organizations: Administrator perspectives

J Fam Violence 2023 Purpose: The COVID-19 pandemic has increased challenges to intimate partner violence (IPV) service provision. This study aimed to explore administrative perspectives on the impacts of the COVID-19 pandemic on United States regional and national IPV service organizations. Methods: We interviewed 35 administrators working within state, regional, or national organizations addressing IPV. Interview domains included (1) organizational response to COVID-19, including communication and supporting employees and partner agencies, (2) impact on marginalized communities, and (3) resource needs. We used a hybrid deductive-inductive approach and thematic analysis for coding and analysis. Results: We identified four key themes: (1) COVID-19 worsened pre-existing challenges and created new challenges at multiple levels within IPV service organizations; (2) IPV service organizations initiated multi-level initiatives to support IPV survivors, their staff, their organization, and their member/partner agencies; (3) Organizations identified changes that should continue beyond the pandemic; and (4) Systemic racism compounded the impact of COVID-19 on IPV survivors and IPV service agencies. Conclusions: Findings suggest that (1) multi-level responses are needed for robust support of IPV survivors during and beyond the pandemic and (2) a syndemic model that addresses underlying structural inequities may strengthen efforts to support IPV survivors during a pandemic or other large-scale disaster. © 2023, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature. |
Adeno-associated virus type 2 in US children with acute severe hepatitis.

Nature 2023 As of August 2022, clusters of acute severe hepatitis of unknown etiology in children have been reported from 35 countries, including the United States(1,2). Previous studies have found human adenoviruses (HAdVs) in the blood from cases in Europe and the United States(3-7), although it is unclear whether this virus is causative. Here we used PCR testing, viral enrichment based sequencing, and agnostic metagenomic sequencing to analyze samples from 16 HAdV-positive cases from October 1, 2021 to May 22, 2022, in parallel with 113 controls. In blood from 14 cases, adeno-associated virus 2 (AAV2) sequences were detected in 93% (13 of 14), compared to 4 (3.5%) of 113 controls (P<0.001) and to 0 of 30 patients with hepatitis of defined etiology (P<0.001). In controls, HAdV-41 was detected in blood from 9 (39.1%) of the 23 patients with acute gastroenteritis (without hepatitis), including 8 of 9 patients with positive stool HAdV testing, but co-infection with AAV2 was observed in only 3 (13.0%) of these 23 patients versus 93% of cases (P<0.001). Co-infections by Epstein-Barr virus (EBV), human herpesvirus 6 (HHV-6), and/or enterovirus A71 (EV-A71) were also detected in 12 (85.7%) of 14 cases, with higher herpesvirus detection in cases versus controls (P<0.001). Our findings suggest that the severity of the disease is related to co-infections involving AAV2 and one or more helper viruses. |

PLOS Glob Public Health 2023 3 (3) e0001252 The first three SARS-CoV-2 phylogenetic lineages classified as variants of concern (VOCs) in the United States (U.S.) from December 15, 2020 to February 28, 2021, Alpha (B.1.1.7), Beta (B.1.351), and Gamma (P.1) lineages, were initially detected internationally. This investigation examined available travel history of coronavirus disease 2019 (COVID-19) cases reported in the U.S. in whom laboratory testing showed one of these initial VOCs. Travel history, demographics, and health outcomes for a convenience sample of persons infected with a SARS-CoV-2 VOC from December 15, 2020 through February 28, 2021 were provided by 35 state and city health departments, and proportion reporting travel was calculated. Of 1,761 confirmed VOC cases analyzed, 1,368 had available data on travel history. Of those with data on travel history, 1,168 (85%) reported no travel preceding laboratory confirmation of SARS-CoV-2 and only 105 (8%) reported international travel during the 30 days preceding a positive SARS-CoV-2 test or symptom onset. International travel was reported by 92/1,304 (7%) of persons infected with the Alpha variant, 7/55 (22%) with Beta, and 5/9 (56%) with Gamma. Of the first three SARS-CoV-2 lineages designated as VOCs in the U.S., international travel was common only among the few Gamma cases. Most persons infected with Alpha and Beta variant reported no travel history, therefore, community transmission of these VOCs was likely common in the U.S. by March 2021. These findings underscore the importance of global surveillance using whole genome sequencing to detect and inform mitigation strategies for emerging SARS-CoV-2 VOCs. |
Molecular survey of Anaplasma and Ehrlichia species in livestock ticks from Kassena-Nankana, Ghana; with a first report of Anaplasma capra and Ehrlichia minasensis.

To combat these effects, it is necessary to identify the circulating pathogens to create effective control measures. This study identified Anaplasma and Ehrlichia species in ticks collected from livestock in the Kassena-Nankana Districts between February 2020 and December 2020. A total of 1550 ticks were collected from cattle, sheep and goats. The ticks were morphologically identified, pooled and screened for pathogens using primers that amplify a 345 bp fragment of the 16SrRNA gene and Sanger sequencing. The predominant tick species collected was Amblyomma variegatum (62.98%). From the 491 tick pools screened, 34 (6.92%) were positive for Ehrlichia and Anaplasma. The pathogens identified were Ehrlichia canis (4.28%), Ehrlichia minasensis (1.63%), Anaplasma capra (0.81%) and Anaplasma marginale (0.20%). This study reports the first molecular identification of the above-mentioned Ehrlichia and Anaplasma species in ticks from Ghana. With the association of human infections with the zoonotic pathogen A. capra, livestock owners are at risk of infections, calling for the development of effective control measures. |
Hierarchical Bayes small area estimation for county-level health prevalence to having a personal doctor

Stat Methods Appt 2022 The complexity of survey data and the availability of data from auxiliary sources motivate researchers to explore estimation methods that extend beyond traditional survey-based estimation. The U.S. Centers for Disease Control and Prevention’s Behavioral Risk Factor Surveillance System (BRFSS) collects a wide range of health information, including whether respondents have a personal doctor. While the BRFSS focuses on state-level estimation, there is demand for county-level estimation of health indicators using BRFSS data. A hierarchical Bayes small area estimation model is developed to combine county-level BRFSS survey data with county-level data from auxiliary sources, while accounting for various sources of error and nested geographical levels. To mitigate extreme proportions and unstable survey variances, a transformation is applied to the survey data. Model-based county-level predictions are constructed for prevalence of having a personal doctor for all the counties in the U.S., including those where BRFSS survey data were not available. An evaluation study using only the counties with large BRFSS sample sizes to fit the model versus using all the counties with BRFSS data to fit the model is also presented. © 2022, Springer-Verlag GmbH Germany, part of Springer Nature. |
Observations from the USA National Phenology Network can be leveraged to model airborne pollen

Aerobiologia (Bologna) 2022 The USA National Phenology Network (USA-NPN) hosts the largest volunteer-contributed collection of plant phenology observations in the USA. The potential contributions of these spatially and temporally explicit observations of flowers and pollen cones to the field of aerobiology remain largely unexplored. Here, we introduce this freely available dataset and demonstrate its prospective applications for modeling airborne pollen in a case study. Specifically, we compare the timing of 4265 observations of flowering for oak (Quercus) trees in the eastern USA to winter–spring temperatures. We then use this relationship to predict the day of peak flowering at 15 pollen monitoring stations in 15 years and compare the predicted day of peak flowering to the peak day of measured pollen (n = 111 station-years). There was a strong association between winter–spring temperature and the presence of open flowers (r2 = 0.66, p < 0.0001) and the predicted peak flowering was strongly correlated with peak airborne pollen concentrations (r2 = 0.81, p < 0.0001). These results demonstrate the potential for the USA-NPN’s phenological observations to underpin source-based models of airborne pollen. Cost analysis of community-based violence prevention programs: Manhood 2.0 and job skills programs

J Fam Violence 2022 Purpose: Sexual violence (SV) and adolescent relationship abuse (ARA) are common in the U. S. and have strong associations with negative health and wellbeing outcomes. Manhood 2.0 is the first U.S. program designed for community settings to build bystander skills while also challenging harmful gender norms. A cluster-randomized trial comparing Manhood 2.0 to Job Skills, a job readiness training control condition, demonstrated that it is a promising strategy to prevent sexual violence and adolescent relationship abuse. Such community-based interventions may be particularly relevant in lower resource urban settings, and the costs of such prevention programs have not been considered previously. Methods: The aim of the present study is to perform systematic and standardized cost calculations associated with implementing Manhood 2.0 among adolescent males. In addition, this study provides detailed cost information of the community-based intervention program, as well as costs associated with implementing the Job Skills control program. Program implementation data were recorded throughout the study period (20152019) by the Manhood 2.0 study team. Results: The cost of implementing Manhood 2.0 is $4,771 per complete round of program delivery and $451 per participant, which is approximately the same cost as the control Job Skills program ($4,432 and $453 per participant). The marginal cost per additional round of Manhood 2.0 program is $3,682. Conclusion: Implementation of a community-based program requires substantial resources and collaborations with community partners especially in economically disadvantaged neighborhoods. This study provides a snapshot of the cost information of a community-based intervention program from the implementing agencys perspective, which is essential in helping decision-makers understand the costs they will incur by implementing prevention programs and ensuring program feasibility and sustainability. 2022, This is a U.S. Government work and not under copyright protection in the US; foreign copyright protection may apply. |

Nature 2022 609 (7925) 101-108 As SARS-CoV-2 continues to spread and evolve, detecting emerging variants early is critical for public health interventions. Inferring lineage prevalence by clinical testing is infeasible at scale, especially in areas with limited resources, participation, or testing/sequencing capacity, which can also introduce biases(1-3). SARS-CoV-2 RNA concentration in wastewater successfully tracks regional infection dynamics and provides less biased abundance estimates than clinical testing(4,5). Tracking virus genomic sequences in wastewater would improve community prevalence estimates and detect emerging variants. However, two factors limit wastewater-based genomic surveillance: low-quality sequence data and inability to estimate relative lineage abundance in mixed samples. Here, we resolve these critical issues to perform a high-resolution, 295-day wastewater and clinical sequencing effort, in the controlled environment of a large university campus and the broader context of the surrounding county. We develop and deploy improved virus concentration protocols and deconvolution software that fully resolve multiple virus strains from wastewater. We detect emerging variants of concern up to 14 days earlier in wastewater samples, and identify multiple instances of virus spread not captured by clinical genomic surveillance. Our study provides a scalable solution for wastewater genomic surveillance that allows early detection of SARS-CoV-2 variants and identification of cryptic transmission. |
Tuberculosis among native Hawaiian and other Pacific Islander persons: United States and U.S.-affiliated Pacific Islands, 2010-2019

Health Equity 2022 6 (1) 476-484 BACKGROUND: In recent years, tuberculosis (TB) incidence in the United States has declined overall but remained high among Native Hawaiian and Other Pacific Islander (NH/PI) persons. Few studies have examined the epidemiology of TB among NH/PI persons, particularly in the U.S.-Affiliated Pacific Islands (USAPI). We describe TB incidence and characteristics of NH/PI patients during 2010-2019. METHODS: We used data from the National Tuberculosis Surveillance System to characterize TB cases reported among NH/PI persons born in the 50 U.S. states (defined to include District of Columbia) and the USAPI. We calculated annual TB incidence among NH/PI patients, stratified by place of birth (U.S. states or USAPI). Using Asian persons born outside the United States-persons historically grouped with NH/PI persons as one racial category-as the reference, we compared demographic, clinical, and socio-behavioral characteristics of NH/PI TB patients. RESULTS: During 2010-2019, 4359 TB cases were reported among NH/PI patients born in the U.S. states (n=205) or the USAPI (n=4154). Median annual incidence per 100,000 persons was 6.5 cases (persons born in the U.S. states) and 150.7 cases (persons born in the USAPI). The proportion of TB patients aged <15 years was higher among NH/PI persons (U.S. states: 54%, USAPI: 24%) than among Asian persons born outside the United States (1%). CONCLUSIONS: TB incidence among NH/PI persons is high, particularly among persons born in the USAPI, emphasizing the need to enhance TB prevention strategies in these communities. Interventions should be tailored toward those who experience the highest risk, including NH/PI children and adolescents. |
MycoSNP: A Portable Workflow for Performing Whole-Genome Sequencing Analysis of Candida auris.

Here, we describe whole-genome sequencing analysis methods used for routine C. auris molecular surveillance in the United States; methods include reference selection, reference preparation, quality assessment and control of sequencing reads, read alignment, and single-nucleotide polymorphism calling and filtration. We also describe the newly developed pipeline MycoSNP, a portable workflow for performing whole-genome sequencing analysis of fungal organisms including C. auris. |
Clinical outcomes associated with SARS-CoV-2 Omicron (B.1.1.529) variant and BA.1/BA.1.1 or BA.2 subvariant infection in southern California.

Nat Med 2022 28 (9) 1933-1943 Epidemiologic surveillance has revealed decoupling of COVID-19 hospitalizations and deaths from case counts following emergence of the Omicron (B.1.1.529) SARS-CoV-2 variant globally. However, assessment of the relative severity of Omicron variant infections presents challenges because of differential acquired immune protection against Omicron and prior variants, and because longer-term changes have occurred in testing and healthcare practices. Here we show that Omicron variant infections were associated with substantially reduced risk of progression to severe clinical outcomes relative to time-matched Delta (B.1.617.2) variant infections within a large, integrated healthcare system in southern California. Adjusted hazard ratios (aHRs) for any hospital admission, symptomatic hospital admission, intensive care unit admission, mechanical ventilation, and death comparing cases with Omicron versus Delta variant infection were 0.59 (95% confidence interval: 0.51-0.69), 0.59 (0.51-0.68), 0.50 (0.29-0.87), 0.36 (0.18-0.72), and 0.21 (0.10-0.44) respectively. This reduced severity could not be explained by differential history of prior infection among cases with Omicron or Delta variant infection, and was starkest among cases not previously vaccinated against COVID-19 (aHR=0.40 [0.33-0.49] for any hospital admission and 0.14 [0.07-0.28] for death). Infections with the Omicron BA.2 subvariant were not associated with differential risk of severe outcomes in comparison to BA.1/BA.1.1 subvariant infections. Lower risk of severe clinical outcomes among cases with Omicron variant infection should inform public health response amid establishment of the Omicron variant as the dominant SARS-CoV-2 lineage globally. |
Simplified Cas13-based assays for the fast identification of SARS-CoV-2 and its variants.

Nat Biomed Eng 2022 6 (8) 932-943 The widespread transmission and evolution of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) call for rapid nucleic acid diagnostics that are easy to use outside of centralized clinical laboratories. Here we report the development and performance benchmarking of Cas13-based nucleic acid assays leveraging lyophilised reagents and fast sample inactivation at ambient temperature. The assays, which we named SHINEv.2 (for 'streamlined highlighting of infections to navigate epidemics, version 2'), simplify the previously reported RNA-extraction-free SHINEv.1 technology by eliminating heating steps and the need for cold storage of the reagents. SHINEv.2 detected SARS-CoV-2 in nasopharyngeal samples with 90.5% sensitivity and 100% specificity (benchmarked against the reverse transcription quantitative polymerase chain reaction) in less than 90 min, using lateral-flow technology and incubation in a heat block at 37 °C. SHINEv.2 also allows for the visual discrimination of the Alpha, Beta, Gamma, Delta and Omicron SARS-CoV-2 variants, and can be run without performance losses by using body heat. Accurate, easy-to-use and equipment-free nucleic acid assays could facilitate wider testing for SARS-CoV-2 and other pathogens in point-of-care and at-home settings. |
Prevalence and genetic characterization of Enterocytozoon bieneusi in children in Northeast Egypt.

Parasitol Res 2022 121 (7) 2087-2092 Enterocytozoon bieneusi is the most common microsporidia in humans worldwide, in addition to infecting a wide range of animals. However, there is limited information about this pathogen in children in Egypt. Here, we carried out a molecular epidemiological study of E. bieneusi in child care centers in three provinces in Egypt. Altogether, 585 fresh fecal samples were collected from children attending 18 child care centers in El-Dakahlia, El-Gharbia, and Damietta provinces in Northeast Egypt during March 2015 to April 2016. PCR and sequence analyses of the ribosomal internal transcribed spacer (ITS) were used to detect and genotype E. bieneusi. Twenty-seven fecal samples (4.6%, 27/585) were positive for E. bieneusi. Five genotypes were identified, including type IV (n = 13), Peru8 (n = 9), Peru6 (n = 2), Peru11 (n = 2), and D (n = 1). Phylogenetic analysis indicated that the five genotypes of E. bieneusi detected in this study were clustered into zoonotic group 1. These data provide important information on the prevalence and genetic diversity of E. bieneusi in children in this country. Further epidemiological studies should be conducted to elucidate the role of zoonotic transmission in human E. bieneusi infections. |
The effect of the body wake and operator motion on the containment of nanometer-scale airborne substances using a conventional fume hood and specially designed enclosing hood: a comparison using computational fluid dynamics

Chemical fume hoods are being utilized as primary controls for worker exposure to airborne substances including nanometer-scale materials due to their overall availability and history of effective contaminant. This study evaluates the impact of the body wake on the containment performance of a conventional constant air volume (CAV) and a new “nano” ventilated enclosing hood using numerical methods. Numerical studies have been performed to predict leaks of nanomaterials handled inside the hood. We further performed experiments in this study to validate the velocity fields predicted by the computational fluid dynamic (CFD) models and to provide a basis for evaluating the impact of the human body on fume hood containment performance. Using these validated models, the effects of the motion of the arms moving out of the hood were simulated using CFD to assess how one of the common actions of an operator/user may affect containment. Results of our simulations show that areas near the hood side airfoils and directly behind the sash are more likely to concentrate contaminants released inside the hood and potentially result in leakage based on internal airflow patterns. These areas are key to monitor when assessing fume hood containment along with the operator/mannequin breathing zone to get an understanding of potential leak areas which might contribute to operator exposure as well as exposure to others inside the laboratory. © 2022, The Author(s), under exclusive licence to Springer Nature B.V. |

To address the public health threat caused by the increasing SARS-CoV-2 genomic diversity, the National Institute of Allergy and Infectious Diseases (NIAID) within the National Institutes of Health (NIH) established the SARS-CoV-2 Assessment of Viral Evolution (SAVE) program. This effort was designed to provide a real-time risk assessment of SARS-CoV-2 variants potentially impacting transmission, virulence, and resistance to convalescent and vaccine-induced immunity. The SAVE program serves as a critical data-generating component of the United States Government SARS-CoV-2 Interagency Group to assess implications of SARS-CoV-2 variants on diagnostics, vaccines, and therapeutics and for communicating public health risk. Here we describe the coordinated approach used to identify and curate data about emerging variants, their impact on immunity, and effects on vaccine protection using animal models. We report the development of reagents, methodologies, models, and pivotal findings facilitated by this collaborative approach and identify future challenges. This program serves as a template for the response against rapidly evolving pandemic pathogens by monitoring viral evolution in the human population to identify variants that could erode the effectiveness of countermeasures. |
Declines in health and support between parents and adult children: Insights from diabetes

Popul Res Policy Rev 2022 41 (4) 1699-1723 We investigate associations between a diabetes diagnosis and financial and instrumental transfers between parents and adult children.Data are from the Panel Study of Income Dynamics, a nationally representative household cohort from the USA with prospective data on diabetes and cross-sectional data on transfers for households in which the head and partner had adult children (n = 4210) or surviving parents (n = 6930). We used survey-adjusted multivariate logistic regressions to compare the probabilities of receiving and giving intergenerational transfers in households where the head and/or partner were recently diagnosed with diabetes or had diabetes-related limitations in daily activities. Households with a diabetes diagnosis in the previous 2years were less likely than those without diabetes to give money to adult children (OR = 0.46, p < 0.01). While recent onset of diabetes was not consistently associated with receiving transfers, transfers were more likely with progressing disease: households in which the head or partner had been diagnose more than 5years earlier were more likely to receive instrumental help from an adult child (OR = 1.24; p < 0.05); those with diabetes-related limitations were more likely to receive assistance, especially instrumental help from adult children (OR = 1.43; p < 0.01) than households without diabetes. The onset of a chronic health condition affects not only individuals own health and financial wellbeing; it also has implications for their adult children and parents, for family relations, time allocation, and financial resources. What does non-standard employment look like in the United States An empirical typology of employment quality

Soc Indic Res 2022 163 (2) 555-583 Despite significant interest in the changing nature of employment as a critical social and economic challenge facing society—especially the decline in the so-called Standard Employment Relationship (SER) and rise in more insecure, precarious forms of employment—scholars have struggled to operationalize the multifaceted and heterogeneous nature of contemporary worker-employer relationships within empirical analyses. Here we investigate the character and distribution of employment relationships in the U.S., drawing on a representative sample of wage-earners and self-employed from the General Social Survey (2002–2018). We use the multidimensional construct of employment quality, which includes both contractual (e.g., wages, contract type) and relational (e.g., employee representation and participation) aspects of employment. We further employ a typological measurement approach, using latent class analysis, to explicitly examine how the multiple aspects of employment cluster together in modern labor markets. We present eight distinct employment types in the U.S., including one resembling the historical conception of the SER model (24% of the total workforce), and others representing various constellations of favorable and adverse employment features. These employment types are unevenly distributed across society, in terms of who works these jobs and where they are found in the labor market. Importantly, women, those with lower education, and younger workers are more likely to be in precarious forms of employment. More generally, our typology reveals limitations associated with binary conceptions of standard vs. non-standard employment, or insider–outsider dichotomies envisioned within dual labor market theories. © 2022, The Author(s), under exclusive licence to Springer Nature B.V. |

Nat Med 2022 28 (5) 1083-1094 The COVID-19 pandemic has demonstrated a clear need for high-throughput, multiplexed, and sensitive assays for detecting SARS-CoV-2 and other respiratory viruses as well as their emerging variants. Here, we present a cost-effective virus and variant detection platform, called microfluidic CARMEN (mCARMEN), that combines CRISPR-based diagnostics and microfluidics with a streamlined workflow for clinical use. We developed the mCARMEN respiratory virus panel (RVP) to test for up to 21 viruses, including SARS-CoV-2, other coronaviruses and both influenza strains, and demonstrated its diagnostic-grade performance on 525 patient specimens in an academic setting and 166 specimens in a clinical setting. We further developed an mCARMEN panel to enable identification of 6 SARS-CoV-2 variant lineages, including Delta and Omicron, and evaluated it on 2,088 patient specimens, with near-perfect concordance to sequencing-based variant classification. Lastly, we implemented a combined Cas13 and Cas12 approach that enables quantitative measurement of SARS-CoV-2 and influenza A viral copies in samples. The mCARMEN platform enables high-throughput surveillance of multiple viruses and variants simultaneously, enabling rapid detection of SARS-CoV-2 variants. |
Sociodemographic attributes and racial disparities for waiting times towards admissions for substance use disorder treatment in the tri-state area (NY-NJ-CT): a cross-sectional study

Int J Ment Health Addict 2022 Given the potential benefits of SUD treatment compliance and the continuing problems associated with waiting for treatment, it is critical to establish an understanding of the factors that influence the cause and duration of waiting time for treatment services.The 2018 US Substance Abuse and Mental Health Services Administration (SAMHSA) Treatment Episode Data Set-Admissions (TEDS-D) were used for this analysis. Prevalence odds ratios from the multivariate logistic regression analyses were used to determine associations between sociodemographic attributes and waiting times prior to SUD admission treatment. Compared to African Americans, the odds of waiting 1–7 days to enter treatment were highest among Alaskan Natives (OR = 1.637, 95% CI: 1.243–2.156). The odds of waiting more than 1 week to enter treatment were 1.690 times higher (95% CI: 1.299–2.197) among Blacks/African Americans after adjusting for race, marital status, employment, and biological sex; there was a statistically significant association between the level of educational attainment and waiting 1–7 days to enter treatment.This study shows that among adults seeking admission into a SUD treatment program, waiting times varies across different sociodemographic attributes especially race as non-Hispanic blacks had a shorter wait time compared to other races. © 2022, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature. |

Enhanced fitness of SARS-CoV-2 variant of concern Alpha but not Beta.

Nature 2021 602 (7896) 307-313 Emerging variants of concern (VOC) drive the SARS-CoV-2 pandemic(1,2). Experimental assessment of replication and transmission of major VOC compared to progenitors are needed to understand successful emerging mechanisms of VOC(3). Here, we show that Alpha and Beta spike (S) proteins have a greater affinity to human angiotensin converting enzyme 2 (hACE2) receptor over the progenitor variant (wt-S(614G)) in vitro. Yet Alpha and wt-S(614G) had similar replication kinetics in human nasal airway epithelial cultures, whereas Beta was outcompeted by both. In vivo, competition experiments showed a clear fitness advantage of Alpha over the progenitor variant (wt-S(614G)) in ferrets and two mouse models, where the substitutions in S were major drivers for fitness advantage. In hamsters, supporting high replication levels, Alpha and wt-S(614G) had comparable fitness. In contrast, Beta was outcompeted by Alpha and wt-S(614G) in hamsters and hACE2-expressing mice. Our study highlights the importance of using multiple models for complete fitness characterization of VOC and demonstrates adaptation of Alpha towards increased upper respiratory tract replication and enhanced transmission in vivo in restrictive models, whereas Beta fails to overcome contemporary strains in naïve animals. |

Methods Mol Biol 2022 2360 279-294 RNA interference (RNAi) is a powerful mechanism that can be exploited not only for physiology research but also for designing insect pest management approaches. Some insects cause harm by vectoring diseases dangerous to humans, livestock, or plants or by damaging crops. For at least a decade now, different insect control strategies that induce RNAi by delivering double stranded RNA (dsRNA) targeting essential genes have been proposed. Here, we focus on nontransgenic RNAi-based approaches that use oral delivery of dsRNA through feeding of inactivated bacteria to produce RNAi in disease vectors and in a crop pest. This potential pest management method could be easily adapted to target different genes or similar organisms. |
