Using regression tree analysis to examine demographic and geographic characteristics of COVID-19 vaccination trends over time, United States, May 2021-April 2022, National Immunization Survey Adult COVID Module

Vaccine 2024 42 (26) 126372 Using data from the nationally representative National Immunization Survey (NIS), we applied conditional linear regression tree methodology to examine relationships between demographic and geographic factors and propensity of receiving various doses of COVID-19 vaccine over time; these analyses identified temporal changes in these relationships that heretofore had not been identified using conventional logistical regression methodologies. Three regression tree models were built using an R package, Recursive Partitioning for Modeling Survey (rpms), to examine propensities over time of receiving a (1) first dose of a two-dose COVID-19 mRNA primary vaccination series or single dose of the Janssen vaccine (vaccine initiation), (2) primary series completion, and (3) monovalent booster dose, using a conditional linear effect model. Persons ≥50 years were more likely to complete a primary series and receive a first booster dose; persons reporting having received non-COVID-19 vaccines recently were more likely to initiate vaccination, complete the primary series, and get a first booster dose; persons reporting having work or school requirements were more likely to complete the primary series. Persons not reporting having received non-COVID-19 vaccines in 2 years but reporting having work or school vaccination requirements were more likely to initiate vaccination than those without work/school requirements. Among persons not reporting having received non-COVID-19 vaccines in 2 years and not reporting having work or school vaccination requirements, those aged ≥50 years were more likely to initiate vaccination than were younger adults. Propensity of receiving various doses was correlated with age, having recently received non-COVID 19 vaccines, and having vaccination requirements at work or school. Regression tree methodology enabled modeling of different COVID-19 vaccination dose propensities as a linear effect of time, revealed changes in relationships over time between demographic factors and propensity of receipt of different doses, and identified populations that may benefit from vaccination outreach efforts. |
Informatics modeling the relationship between proxy measures of respondent burden and survey response rates in a household panel survey
Earp M , Kaplan R , Toth D . J Off Stat 2022 38 (4) 1145-1175 Respondent burden has important implications for survey outcomes, including response rates and attrition in panel surveys. Despite this, respondent burden remains an understudied topic in the field of survey methodology, with few researchers systematically measuring objective and subjective burden factors in surveys used to produce official statistics. This research was designed to assess the impact of proxy measures of respondent burden, drawing on both objective (survey length and frequency), and subjective (effort, saliency, and sensitivity) burden measures on response rates over time in the Current Population Survey (CPS). Exploratory Factor Analysis confirmed the burden proxy measures were interrelated and formed five distinct factors. Regression tree models further indicated that both objective and subjective proxy burden factors were predictive of future CPS response rates. Additionally, respondent characteristics, including employment and marital status, interacted with these burden factors to further help predict response rates over time. We discuss the implications of these findings, including the importance of measuring both objective and subjective burden factors in production surveys. Our findings support a growing body of research suggesting that subjective burden and individual respondent characteristics should be incorporated into conceptual definitions of respondent burden and have implications for adaptive design. © 2022 Morgan Earp et al. |
Delivery of an HIV prevention counseling program in an infectious diseases clinic: implementation process and lessons learned

AIDS Patient Care STDS 2009 23 (6) 433-41 Current national guidelines recommend that all HIV care providers routinely counsel their HIV-infected patients about reducing HIV transmission behaviors. In this article we identify the challenges and lessons learned from implementing a provider-delivered HIV transmission risk-reduction intervention for HIV-infected patients (Positive Steps). Based on a multi-site Centers for Disease Control and Prevention (CDC) initiative, we integrated the Positive Steps program into an infectious diseases clinic in North Carolina. Of the nearly 1200 HIV-infected patients, 59% were African American, 44% were white, 33% were women, and over 50% were between 25 and 44 years of age. We obtained feedback from a community advisory board, input from clinic staff, and conducted formative interviews with clinic patients and providers to achieve overall acceptance of the program within the clinic. Clinic providers underwent training to deliver standardized prevention counseling. During program implementation we conducted a quality assessment of program components, including reviewing whether patients were screened for HIV transmission risk behaviors and whether providers counseled their patients. Once Positive Steps was implemented, on average, 69% of patients were screened and 77% of screened patients were counseled during the first 12 months. In analyses of quarterly exit surveys of patients after their medical exams, on average, 73% of respondents reported being asked about safer sex and 51% reported having safer-sex discussions with their providers across six quarterly periods. Of those who had discussions, 91% reported that those discussions were "very" or "moderately helpful." Providers reported time and competing medical priorities as barriers for discussing prevention with patients, however, provider-delivered counseling was routinely performed for 12 months. Overall, the findings indicate that the Positive Steps program was successfully integrated in an infectious diseases clinic and received well by patients. |
